Pemi-Baker Hospice & Home Health Announces Upcoming Hospice Volunteer Training.

Volunteer delivering groceries, Pemi-Baker Hospice & Home Health, Plymouth, NHPemi-Baker Hospice & Home Health is excited to announce their upcoming Hospice Volunteer Training, providing an opportunity for community members to become hospice volunteers and experience the rewarding journey of giving back to those in need.

Volunteering has proven benefits, including stress reduction, combating depression, mental stimulation, and a sense of purpose. Pemi-Baker Hospice recognizes the invaluable impact of volunteers in supporting families and individuals during challenging times.

Our dedicated volunteers come from diverse professional backgrounds, representing the 32 towns served by Pemi-Baker. Many volunteers have a personal connection to someone who has benefited from hospice care, driving their passion to contribute to the well-being of other families in their community.

In response to patient requests, Pemi-Baker is also actively seeking male volunteers and individuals with a military background. This initiative aims to expand the range of support available to our Veteran patients, ensuring a more comprehensive and tailored approach to their unique needs.

No matter your background, hospice volunteers play a crucial role in supporting families in various ways, including reading to patients, writing letters, relieving family caregivers, light housekeeping, assisting with outside chores, walking dogs, or cooking meals. Regardless of the time commitment, whether it’s 1-2 hours per week or per month, every contribution is valuable.

Pemi-Baker Hospice & Home Health offers free Hospice Volunteer training, Plymouth, NHThe Hospice Volunteer Training program is a 5-week session scheduled from 2-4 pm on Tuesdays, starting February 13th through March 12th. The training is free of charge and open to anyone interested in making a meaningful difference in the lives of those facing end-of-life challenges.

November is Home Care & Hospice Month

More and more, people prefer to be cared for in the comfort and dignity of their own homes. Pemi-Baker Hospice & Home Health provides in-home medical services that are cost-effective and person-centered. November is a chance to recognize the dedication and innovation of providers of health care at home. It is an opportunity to acknowledge recipients of in-home care services, and celebrate that they are able to remain in the safety and comfort of their own homes while receiving quality care.

Home health, hospice & palliative care providers have always been important to vulnerable citizens in our communities. But now that importance is underscored by the risk of exposure to COVID-19 in public/facility settings. When given the option, patients want to receive care in their homes. We believe that desire is stronger today than it has ever been. Pemi-Baker Hospice & Home Health is proud to support 29 communities in central and northern NH.

Home Health

Home Health provides skilled services, including nursing care, physical, occupational and speech therapy. Pemi-Baker will send a licensed professional to assess home health needs and develop a complete care plan. These services are usually short-term, as in the case of a person recovering from surgery or other health issues.

What are the costs and what is covered?
Medicare will cover home health if it is ordered by a physician and the person is homebound and in need of skilled services. Services are usually covered for a 30-day “episode of care.” However, if an individual is enrolled in a Medicare HMO, known as a Medicare Advantage Plan, services are authorized on a per-visit basis. Long-term care insurance and Medigap plans may also cover some costs of a home health aide.

Hospice Care

Hospice care brings together medical care, pain management and emotional and spiritual support for patients at the end of life and their families. The mission of hospice staff and volunteers is to address the symptoms of a terminal illness with the intent of promoting comfort and dignity. They are experts at pain management.

Hospice will manage your loved one’s pain, help you understand what he or she is going through and help you and other family members cope with your emotions. Many of us are at a loss as to what we should do before and at the time of death. Hospice professionals help you get through a very difficult time and are there even after your loved one’s death.

What are the costs and what is covered?

Medicare and Medicaid provide a hospice benefit that covers almost all of the costs of caring for a dying person during his or her last six months of life. To qualify for the Medicare hospice benefit, a person must have:

  • Medicare Part A
  • A life expectancy of less than six months
  • An agreement in writing that he or she will not pursue any treatments to cure his or her illness

The Medicare hospice benefit covers skilled nursing services, physician visits, skilled therapy (i.e. physical, speech or occupational), medical social services, nutrition counseling and bereavement counseling, most of the cost of prescription drugs for symptom control and pain relief. Short-term inpatient respite care to relieve family members from care-giving is also covered.

Pemi-Baker Hospice & Home Health Celebrates Gazebo Dedication

The sun shone brightly at Pemi-Baker Hospice & Home Health’s special gazebo dedication to the Pemi-Baker staff in memory of Frances Marie Fernandes, a former hospice patient. John Fernandes and his two daughters have been long supporters of the Pemi-Baker Hospice and Palliative care programs but they wanted to give back specifically to the staff, giving them a place to rest, reflect and rejuvenate. 

The ornate, covered gazebo rests on the lawn outside the offices at Boulder Point, overlooking the pond and mountains to the north of Pemi-Baker Valley. “Fran would want you to enjoy this gazebo because it is a link to the beauty of nature. Maybe on a pleasant day, sitting here and contemplating the beautiful scenery, you will feel something of the appreciation our family has for what you do. We hope you will continue to enjoy this place for many years to come,” said John Fernandes.

The staff at Pemi-Baker would like to gratefully thank the whole Fernandes family: John Fernandes, Laura Kelly, Leslie & Dan Keane and Richard Cusolito for this amazing gift and thoughtful tribute. PBH&HH Score High in Hospice Satisfaction Survey Results

According to the most recent independent Strategic Healthcare Program (SHP) report comparing agencies of the Rural Health Care Network (RHCN), Pemi-Baker Hospice & Home Health (PBH&HH) shines bright in all categories and is at the top or tied for the top in over 20 categories of patient satisfaction for Hospice. We are touched and honored that our hospice families gave us such high scores with regard to the care their loved ones received.

The report measures many details about the agency’s interactions and care including communication with the family, getting timely help, treating patients with respect, emotional and spiritual support, help for pain and symptoms and training the family to care for the patient. Though PBH&HH scored high on all survey questions the following items scored a 100%-actual positive score:

  • “Communication with the family”-composed of separate questions, including-
  1. Things were explained in a way that was easy to understand,
  2. Hospice team listen carefully about any problems with care,
  3. Family kept informed about patient’s condition
  • “Treating the patient with respect”-composed of 2 separate items including-
  1. How often patient was treated with dignity and respect
  2. You felt the hospice team really cared about the patient
  • “Percentage of families who would rate the Hospice care as a 9 or 10 on a 0-10 scale”
  • “Would you recommend this Hospice to family or friends?”

Pemi-Baker Hospice & Home Health Raises Critical Funds for Patients Through Online Auction

Pemi-Baker Hospice & Home Health (PBH&HH) celebrated another successful fundraising auction this spring thanks to the generosity of community sponsors, donors and bidders.

With over 125 items to bid on and specialty services to donate towards, they hit their fundraising goal and raised the critical funds needed to continue their important work in supporting the members of 29 communities who need medical aid at home. All proceeds from the auction support patients and families with little or no health insurance. Some of the auction donations are specific to palliative and hospice services including: therapeutic massage, hospice and dementia comfort cubs, therapeutic music and hospice comfort kits.

Why We Love National Home Health & Hospice Month

National Home Care and Hospice Month in November is when we honor all professionals who work in the broad field of home care and hospice work – this includes physicians, nurses, LNAs, social workers, physical therapists, and so many more. Each requires a different skill-set and comes with its own emotional and physical demands. The professionals who perform these services are committed to helping patients live, as well as pass away with dignity. This month is for them.

  1. Home Health Care Can Reduce Falls and Hospital Re-admissions

If you have had surgery or been hospitalized for a medical illness, you might have safety concerns at home, particularly the risk of falls. The truth is that falls with resulting injuries are quite common in hospitals, despite efforts to reduce them. There is a growing body of evidence to suggest that an elderly or ill person is less likely to suffer a serious event at home compared to an acute care setting. Home care can also reduce the rate of hospital re-admission.

  1. Home Health Care Provides a Medical Level of Care at Home

After hospitalization, most people prefer to heal at home. Similarly, the elderly prefer to stay home rather than go to a skilled nursing facility.

Home health care can provide a variety of benefits to support activities of daily living as well as 24/7 nursing care, disease management, ostomy and catheter management, surgical after care/wound care, medication management and care for high risk pregnancies, infants and children.

 

If the only thing keeping a patient in the hospital is the need for intravenous medications, there is good news: there is now a way to manage infusion therapy at home.

A therapist’s trained eyes can spot fall hazards or bathroom dangers and provide helpful preventive advice.

  1. Hospice Provides Dignity for Many

Home care and hospice workers believe that those at the end of their lives deserve the right to pass peacefully and on their own terms. The elderly often far prefer to stay in the comfort of their homes, where they feel freer and are at less risk of contracting other illnesses. The existence of hospice makes this level of freedom available to many who otherwise would not have had it. Pemi-Baker offers advice and support in the home to the families they are caring for and also offers a Caregiver Support group every first Wednesday of the month at 10:00am in person or via ZOOM.

Dr. Kristi Saunders Joins Pemi-Baker Community Health

Pemi-Baker Community Health is excited to announce that Dr. Kristi Saunders, MD, has joined the team as their new Hospice Medical Director.

After working over twenty years in Family Medicine as a board certified OB/GYN, Dr. Saunders was drawn to palliative care and hospice by the desire to be more involved with her patients when facing serious illnesses and death. She became board certified in Hospice and Palliative Medicine in 2012 and since then has been educating her patients about living and embracing the normalcy of dying.

Working with other agencies in New Hampshire and recently receiving hospice help for her mother in California, has highlighted common challenges that families face. “My past experiences of different cultures, needs and desires made me realize how important it is to really have the time to listen, watch, and not treat everyone like medicine normally does…like everyone else. We are all different and need our care individualized. We spend too much time medicalizing death and not spending more time in what is the hard part…saying goodbye,” said Saunders. Pemi-Baker Community Health’s mission and philosophy of supporting their patients and families with compassionate care follows closely with Dr. Saunders’ beliefs. “Our goal is to educate families that hospice is really about living, not death. It should be about smiles, hugs and bringing people closer to acceptance as we possibly can,” said Saunders.

“Dr. Saunders has been serving our community as one of our hospice physicians since January and we are delighted that she will be taking over as Hospice Medical Director upon Dr. Arsenault’s retirement. Her commitment to the highest standards of client care blended with her knowledge and compassion make her an ideal fit for Pemi-Baker Community Health,” said Danielle Paquette-Horne, Senior Director of Home Health, Palliative Care & Hospice.

The Rewards of Being a Hospice Volunteer

While I was familiar with Hospice for many years, I only have been a hospice volunteer with Pemi Baker Hospice and Home Health (PBHHH) since October 2021.  When I retired, I began searching for volunteer work that would allow me to give back to my community.  A newsletter from PBHHH arrived in my email announcing training for hospice volunteers.  My family had experienced the need for hospice twice, once in Ohio and again in Florida.  The relief my family and I felt when the hospice volunteers arrived was beyond measure.  I want to bring the same kind of assistance to others.  PBHHH given me that opportunity by providing professional training with knowledgeable nurses, doctors, social workers and other health care professionals.  Their guidance has enabled me to meet the challenges of being s hospice volunteer, and I am very glad that I did it.
