How to Plan For Recovery at Home After Surgery

Although it’s not always possible to schedule your surgery in advance, you should understand beforehand how long your recovery may take, what you will and will not be able to do, and what help you will need while you are home.

Speaking to your doctor or your hospital discharge planner about home health care services prior to surgery will help you prepare for when you go home again. If your surgery is the result of an emergency situation, a family member or other caregiver could arrange for services on your behalf.

What you can do ahead of the surgery

You can make your recovery at home go a lot smoother if you take a little time to plan ahead. Ask your doctor what you won’t be able to do at first while you’re healing up, and get help from friends and family to prepare your living space. You need to be careful about falling after an operation. “People get dizzy because they haven’t eaten for a few days and from being in bed,” says Jenn Stewart, RN, Clinical Supervisor. “You may need to get a walker or crutches to cut down your risk of a fall. Other tips to avoid stumbling would be using night lights in hallways, sleeping in a bedroom near a bathroom, wearing flat shoes or slippers and reducing the clutter in your home” said Stewart.

  • Do I need to make changes to my house? This depends on what kind of operation you have. If it’s a complicated one, like a joint replacement or stomach surgery, you may need to tweak a few things in the bathroom or bedroom for easier access.
  • Stairs: If you can’t climb up and down them after your surgery, you might have to make some changes. For instance, if your bedroom is upstairs, you may need to sleep on a lower floor for a while. Ask your friends or family to move your bed before you go into the hospital, or check into renting a hospital bed if you need it.
  • Equipment: After some types of surgery, you’ll need to get special gear at home. Plan ahead if your doctor tells you that you need oxygen tanks, elevated toilets, shower seats, supplies to care for your cut, or other items. Get in touch with your insurance company to see if they’re part of your coverage.
  • Stock your pantry: Ask your doctor if you have to stick to a certain diet. Make sure you’re well supplied with the right food before you get back home. Since you may feel tired during your recovery, prepare some meals before your operation and put them in the freezer.

There are many things you and your family can do to prepare for your surgery and give you the best possible recovery. More and more, people prefer to be cared for in the comfort and dignity of their own homes. Pemi-Baker Hospice & Home Health provides in-home medical services that are cost-effective and person-centered. November is a chance to recognize the dedication and innovation of providers of health care at home. It is an opportunity to acknowledge recipients of in-home care services, and celebrate that they are able to remain in the safety and comfort of their own homes while receiving quality care.

Home health, hospice & palliative care providers have always been important to vulnerable citizens in our communities. But now that importance is underscored by the risk of exposure to COVID-19 in public/facility settings. When given the option, patients want to receive care in their homes. We believe that desire is stronger today than it has ever been. Pemi-Baker Hospice & Home Health is proud to support 29 communities in central and northern NH.

Home Health

Home Health provides skilled services, including nursing care, physical, occupational and speech therapy. Pemi-Baker will send a licensed professional to assess home health needs and develop a complete care plan. These services are usually short-term, as in the case of a person recovering from surgery or other health issues.

What are the costs and what is covered?
Medicare will cover home health if it is ordered by a physician and the person is homebound and in need of skilled services. Services are usually covered for a 30-day “episode of care.” However, if an individual is enrolled in a Medicare HMO, known as a Medicare Advantage Plan, services are authorized on a per-visit basis. Long-term care insurance and Medigap plans may also cover some costs of a home health aide.

Hospice Care

Hospice care brings together medical care, pain management and emotional and spiritual support for patients at the end of life and their families. The mission of hospice staff and volunteers is to address the symptoms of a terminal illness with the intent of promoting comfort and dignity. They are experts at pain management.

Hospice will manage your loved one’s pain, help you understand what he or she is going through and help you and other family members cope with your emotions. Many of us are at a loss as to what we should do before and at the time of death. Hospice professionals help you get through a very difficult time and are there even after your loved one’s death.

What are the costs and what is covered?

Medicare and Medicaid provide a hospice benefit that covers almost all of the costs of caring for a dying person during his or her last six months of life. To qualify for the Medicare hospice benefit, a person must have:

  • Medicare Part A
  • A life expectancy of less than six months
  • An agreement in writing that he or she will not pursue any treatments to cure his or her illness

The Medicare hospice benefit covers skilled nursing services, physician visits, skilled therapy (i.e. physical, speech or occupational), medical social services, nutrition counseling and bereavement counseling, most of the cost of prescription drugs for symptom control and pain relief. Short-term inpatient respite care to relieve family members from care-giving is also covered.

Pemi-Baker Hospice & Home Health Raises Critical Funds for Patients Through Online Auction

Pemi-Baker Hospice & Home Health (PBH&HH) celebrated another successful fundraising auction this spring thanks to the generosity of community sponsors, donors and bidders.

With over 125 items to bid on and specialty services to donate towards, they hit their fundraising goal and raised the critical funds needed to continue their important work in supporting the members of 29 communities who need medical aid at home. All proceeds from the auction support patients and families with little or no health insurance. Home Care: More Than Just a Visiting Nurse

When patients leave the hospital and return home, they go from highly supportive medical environments with physicians, nurses, aides, and other professionals, to non-medical environments with informal caregiver support. If You Live With a Serious Illness, Learn How to Improve Your Quality of Life

If you have cancer, lung, heart or kidney disease, Alzheimer’s, or another serious illness, you should know about a medical team that can address your symptoms and stress.

The medical team is called palliative care and they’re available in a variety of medical settings. Pemi-Baker Community Health (PBCH) offers palliative care in your home, making it easy for you and your family members to ask questions and relay concerns. Their focus is on relieving the symptoms and stress of your illness. The goal is to improve quality of life for both the patient and the family. This is done by coordinating the patient’s care as well as providing support for the caregivers if they are experiencing caregiver burnout.

Pemi-Baker Community Health’s palliative care is provided by a specially trained team of doctors, nurses and other specialists who work together with your other doctors to provide an extra layer of support. Palliative care is appropriate at any age and at any stage in your illness, and it can be provided along with curative treatment.

Is Palliative Care right for you?

Answer the following questions to determine whether palliative care might be right for you or someone close to you. Remember, you can receive palliative care at any point in your illness.

  1. Do you have one or more serious illnesses such as:
  • Cancer
  • Congestive heart failure (CHF)
  • Chronic obstructive pulmonary disease (COPD), emphysema, lung disease
  • Kidney failure
  • Liver failure
  • Neurological diseases (e.g., ALS, Parkinson’s)

  • Difficult side effects from treatment
  • Eating problems due to a serious illness
  • Frequent emergency room visits
  • Three or more admissions to the hospital within 12 months, and with the same symptoms
  1. Do you, or someone close to you, need help with:
  • Knowing what to expect
  • Knowing what programs and resources are available
  • Making medical decisions about treatment choices/options
  • Matching your goals and values to your medical care
  • Understanding the pros and cons (benefits/burdens) of treatments (e.g., dialysis, additional cancer treatments, surgery, etc.)
  1. Do you, or someone close to you, need help with:
  • Coping with the stress of a serious illness
  • Emotional support
  • Spiritual or religious support
  • Talking with your family about your illness and what is important to you

If you answered yes to any of the questions above, you can benefit from palliative care. Ask your doctor for a palliative care referral with Pemi-Baker Community Health. Once PBCH receives the referral from your doctor they will contact you via phone to answer and ask questions and set up a meeting with you at your home.

What happens at a first visit with a palliative care team?

The first step is for the palliative care team to get to know you as a person and find out who else is important in your care. They will want to hear your narrative – who you are, what you enjoy doing, and what is most important to you in your day-to-day life. They’ll review your medical information, ask what symptoms are of concern, and how much these affect your quality of life. They’ll ask questions to see if there are any care needs at home. They may also ask questions to learn what you know about your medical condition, and what concerns you about the future so they can plan how to help over time. The team will also ask questions about who you would want to help make decisions if you aren’t able to do so for yourself. This person is called a health care agent or proxy.

Advance Care Planning-An important home health service you shouldn’t overlook

Have you wondered how to talk to your loved ones about what medical care you would or wouldn’t want in a medical crisis?  Does your health care provider want you to have these conversations and fill out your end of life wishes?  Would your wishes change if it was COVID-19 related?  Pemi-Baker Community Health can help with all your advance care planning needs.

Advance care planning (ACP) is a process of understanding, reflecting on and discussing your experiences, values and beliefs in an effort to identify how you want to live if faced with a serious or life-limiting illness. It may include identifying the care you would want at the end of your life or the care you would want if you were too injured or ill to speak.

It is also important to choose someone to speak on your behalf if you are unable to make your own decisions, and take comfort in knowing that person will convey your wishes. This person is called a Health Care Proxy, in NH it is termed Durable Power of Attorney for Healthcare.  It could be your partner, a sibling or a close friend.

Once you decide what care you want and who to help voice your opinions, you will be ready to complete an ACP document, often referred to as an advance directive. This is the legal form from your state that puts your wishes on paper and legalizes them with witness signatures. When this form is complete, you will want to give it to and communicate your wishes to your family, friends, clergy, other advisors, physicians and other health care professionals to help ensure that your wishes are honored.

Skilled Home Health Care: What Pemi-Baker Community Health Can Do For You

November is National Homecare Month. What better way to celebrate it than to provide information about home care services thru Pemi-Baker Community Health, and what it can offer for a loved one? As we age, we still want our independence. Many older Americans are choosing to stay in their homes longer and “age in place.” This trend allows people to remain in their family home surrounded by familiar furnishings that make them feel comfortable and safe. But as we get older, certain activities that once seemed simple can become more difficult to manage.

So when do we need skilled home health care?

Skilled home health is care given to patients at home by medically trained and licensed professionals. It is an alternative to receiving care in a nursing home or hospital to regain health and independence.

What sort of people are served by home health?

Skilled home health care clients can be recovering patients who are ill with a chronic condition like heart failure or an acute health condition like surgery for a hip replacement, or who have sustained an injury. Home health care permits earlier discharge from a hospital, reduces hospital admissions and assists with rapid recovery. It’s less expensive, more accessible, and as effective as care from a hospital or skilled nursing facility.

For example, your Mom might need skilled nursing after being hospitalized with a bout of pneumonia or Dad may need occupational and physical therapy when he gets home following knee surgery. The nurse can help your Mom learn to manage her medications safely and help her learn ways to manage her overall health more effectively. Occupational and physical therapy will provide training on the best exercises Dad can do to strengthen his knee so he can get back to doing the things he enjoys.

Long-term home health palliative or hospice services can be provided when your parent or loved one is suffering with a terminal illness. Our nurses and therapists work together with your doctors to provide coordinated, effective care.

How does a patient access skilled home health thru Pemi-Baker Community Health?

A referral from a physician, nurse practitioner, or physician’s assistant is necessary. If your loved one is being sent home from the hospital or nursing home, their staff physicians will typically write the referral. Your own PCP can also provide a referral.  Just let them know you would like to use Pemi-Baker Community Health, we service 22 towns in Central New Hampshire.

Who pays for skilled home health care?

Payment options for home health typically include a wide variety of insurances including Medicare, many Medicare Replacement plans, along with a whole host of private insurances.

No-Show Auction a Success!

Pemi-Baker Community Health Raises Critical Funds for Home care and Hospice Patients-

The COVID-19 pandemic put a damper on Pemi-Baker Community Health’s plans for their Annual Auction and Brunch fundraiser but thanks to the generosity of community sponsors, donors and bidders, their ‘No-Show Auction’, an on line only event, was a success!

With over 85 items to bid on, raffles to invest in and specialty services to donate towards, they hit their fundraising goal and raised the critical funds needed to continue their important work in supporting the members of this community who need medical aid at home. Now, more than ever, PBCH patients need nursing and social work support for their sudden illnesses and hospice needs. Many of their patients are experiencing increased feelings of depression and isolation, as well as heightened anxiety, particularly because many are at high-risk for COVID-19.

Home health care is becoming a new front in the national fight against COVID-19 as hospitals discharge patients home and others strive to stay out of them. The World Health Organization just named 2020 the year of the Nurse and Midwife and with National Nurses week right upon us, Pemi-Baker would like to say thank you to all of our Nurses and Licensed Nurse Assistants who go above and beyond, day in and day out.

Home care nurses, therapists, and aides — who normally help an estimated 12 million Americans with everything from bathing to IV medications — are now taking on the difficult and potentially dangerous task of caring for coronavirus patients. While Americans are being told to keep to themselves, home health providers and their clients still largely have to engage in person, often in close proximity. Many agencies are ramping up phone or video visits but these are typically not covered by insurance and even the smartest phone can’t listen to someone’s lungs or get them to the bathroom.

The crisis is testing the industry, but it’s also a moment of pride for workers who have often felt under-recognized.  “We have taken the Covid 19 pandemic as an opportunity,” said Danielle Paquette-Horne, PBCH Home Health, Palliative and Hospice Care Director. “We’re getting a chance to establish what can be done different by modifying our daily operations and to show the community that they can continue to count on us to serve them and their family during this pandemic,” Paquette-Horne said.

Coronavirus care at home has expanded rapidly in the last few weeks. Some agencies in most states are now taking COVID-19 patients referred after hospitalization or nursing home care or as an alternative to them, and Pemi-Baker Community Health is rising to the challenge. 

Pemi-Baker Community Health’s COVID-19 taskforce meets three times a week to assure Pemi-Baker is up to date with CDC guidelines and like their colleagues in hospitals and nursing homes, PBCH’s home care workers have faced a scarcity of protective equipment.

“We are very grateful to the EVERSOURCE Foundation for their recent grant to help us buy more protective supplies for our nurses, LNAs and patients,” said Chandra Engelbert, Pemi-Baker Community Health, CEO.  “To have a community partner we can rely on when our non-profit faces these challenges, is crucial and priceless,” said Engelbert, “Countless Pemi-Baker friends have also sewn cloth face masks for our staff and patients, (double prevention), and we want to thank them as well.”

The Rewards of Being a Hospice Volunteer

While I was familiar with Hospice for many years, I only have been a hospice volunteer with Pemi Baker Hospice and Home Health (PBHHH) since October 2021.  When I retired, I began searching for volunteer work that would allow me to give back to my community.  A newsletter from PBHHH arrived in my email announcing training for hospice volunteers.  My family had experienced the need for hospice twice, once in Ohio and again in Florida.  The relief my family and I felt when the hospice volunteers arrived was beyond measure.  I want to bring the same kind of assistance to others.  PBHHH given me that opportunity by providing professional training with knowledgeable nurses, doctors, social workers and other health care professionals.  Their guidance has enabled me to meet the challenges of being s hospice volunteer, and I am very glad that I did it.
