Home Health Payment Information
Pemi-Baker Community Health is a nonprofit organization, Medicare and Hospice certified, and licensed by the State of New Hampshire.
- We bill Medicare, Medicaid, and private and third party insurances.
- We are a NH Licensed Home care facility and must meet licensure requirements annually.
- In addition, we are surveyed by Medicare, Medicaid Grafton County, and the State of NH Adult & Elderly Services.
- If care is medically necessary and the patient meets certain coverage requirements, Medicare, Medicaid, and most private insurance will pay for home care. Please contact us to speak with someone about your payment options.
ReferralsReferrals for Pemi-Baker Home Health services can come from a variety of sources, including prospective patients, their family members or friends, hospital discharge planners, physicians, community health and social service agencies.
InsuranceBecause insurance coverage can sometimes change, please contact us to discuss payment and insurance options.
EvaluationIf needed, a Pemi-Baker nurse may make a no-charge evaluation to determine if services might be beneficial.
Choices for Independence ProgramNew Hampshire is committed to providing home and community based services that promote independence, safety and dignity. Since 1967, Pemi-Baker Community Health has been a provider of skilled, compassionate, progressive home-based health services to our community members. Many medical services that were once only available in a hospital or rehabilitation facility are now safely and effectively administered by our staff who are experts in home care.
