New Grief Support Group ‘Enduring Through Loss’ Starting

December 19, 2024

Pemi-Baker Hospice & Home Health Offers New Grief Support Group ‘Enduring Through Loss’.

As part of its service to our wider community, PEMI-BAKER HOSPICE & HOME HEALTH will be offering a grief and loss support group, beginning on Tuesday, January 14, and continuing through February 18, 2025.  This six-week series “Enduring Through Loss” will explore the experiences of grieving and mourning.  Individuals who have been bereaved can often feel isolated and alone, unequipped to deal with the emotional upset and turbulence following a loss.  Sometimes they feel they are coming unhinged and losing control of their thoughts and behaviors. Grief & Loss support group at Pemi-Baker Hospice & Home Health

This series will seek to validate and normalize some of the various reactions and responses characteristic of grief, while coming to terms with a loss.  Suggestions toward coping skills and ways to quell and soften the intensity of the personal and individual features of one’s experience will be part of the support offered.

The experience of loss is not restricted to the death of a loved one, but may include the loss of employment, the severing of a relationship, financial setbacks, or the effects of a natural disaster, or relocation to an unfamiliar area or setting.

The group will meet on Tuesday mornings at 10:30am in the upstairs Art Room at the Plymouth Regional Senior Center.  Each session will conclude by 12:00 noon.  Unfortunately, the elevator at the Senior Center is currently out of order, so participants need to be able to use the stairs to access the Art Room.

There is no cost to participate in the group.  To know how many to prepare for, participants need to pre-register by contacting Guy Tillson, Hospice Chaplain and Bereavement Coordinator at Pemi-Baker Hospice & Home Health.  He can be reached by email at gtillson@pbhha.org or by phone at 603 536-2232.

Protecting Older Adults from Fall-Related Injuries

September 8, 2024

Pemi-Baker Hospice & Home Health Celebrates Fall Prevention Month This September- Raising Awareness and Taking Action to Protect Older Adults from Fall-Related Injuries.

September is Fall Prevention Month, a critical time to focus on the safety and well-being of our older adult population. Pemi-Baker Hospice & Home Health is proud to join this national effort to raise awareness about fall-related injuries, hospitalizations, and their significant impact on quality of life and healthcare costs.Reducing the risk of falls in the home, Pemi-Baker Hospice & Home Health, Plymouth, NH

According to the Centers for Disease Control and Prevention (CDC), one in four Americans aged 65 and older falls each year. Falls are the leading cause of both fatal and non-fatal injuries in this age group, leading to over 3 million emergency department visits annually. Beyond the physical injuries, falls can result in a reduced quality of life, with many older adults experiencing a loss of independence and increased fear of falling again.

The financial impact is also staggering. The CDC estimates that falls among older adults cost the U.S. healthcare system over $50 billion each year. These costs include hospitalizations, surgeries, and long-term rehabilitation, which often place a significant burden on families and caregivers.

Preventing Falls at Home: Simple Steps for Safety

Many falls can be prevented with simple adjustments to the home environment and proactive health measures. Pemi-Baker Hospice & Home Health encourages individuals and families to consider the following fall prevention strategies:

  • Remove Tripping Hazards: Keep walkways clear of clutter, secure loose rugs with double-sided tape, and immediately clean up spills.
  • Improve Lighting: Ensure all areas of the home are well-lit, especially stairways and bathrooms. Consider installing nightlights in hallways and bedrooms.
  • Install Grab Bars and Handrails: Place grab bars in the bathroom near the toilet and shower, and install handrails on both sides of stairways.
  • Wear Proper Footwear: Encourage the use of shoes with non-slip soles and avoid walking around the house in socks or slippers.
  • Exercise Regularly: Engage in exercises that improve balance and strength, such as Tai Chi, to help prevent falls.

In-Home Assessments by Our Experts

At Pemi-Baker Hospice & Home Health, our team of Physical and Occupational Therapists is dedicated to helping older adults stay safe in their homes. We offer in-home assessments to identify potential fall hazards and provide personalized recommendations to enhance safety. Our therapists work closely with patients to develop tailored exercise plans that strengthen muscles and improve balance, further reducing the risk of falls.

“As we celebrate Fall Prevention Month, it is vital to recognize the profound impact falls have on older adults and their families,” said Aubrey Engle, RN, Executive Director of Pemi-Baker Hospice & Home Health. “By taking proactive steps, we can prevent falls and ensure our loved ones maintain their independence and quality of life. Our team is here to support the community with expert assessments and care designed to keep everyone safe at home.”

Don’t wait to ask about how our compassionate staff can help your loved ones. To qualify for home health care, a person must have a physician’s order and demonstrate a need for skilled care that can be safely provided at home. The Journey Through Loss & Grief-New Support Group Starting!

August 26, 2024

Pemi-Baker Hospice & Home Health Starting New Grief Support Group Session: ‘The Journey Through Loss and Grief’

Throughout the ages of humankind, storytellers have spun tales which have fascinated the minds and imaginations of folks young and old as ways to help them discover their own inner strengths to confront the difficulties of life as we come to experience them. Probably no emotional journey is more challenging and difficult than that of loss and the feelings of grief that accompany it.

We do not have to wait long for such experiences. The boy next door, out of innocent curiosity drops a rock on a plastic rattle belonging to the little girl upstairs to see what makes its noise. The girl has lost her toy. The boy gets punished. Years move on. There are fun things that happen, but then the boy doesn’t make the softball team, or the girl doesn’t get the part in the play. Life moves on. She wins a prize in field hockey. Pets die. Boys or girls we develop crushes on don’t feel the same affection for us. Then we leave home to find our own paths in life. And on and on and on and on.

In between the good things, there are other things that hurt us, and we make our way on our journey, most of the time probably not knowing that we’re learning how to handle our losses and composing our own personal story. And then someone truly special we love is lost to us- by natural death, an accident, or by a violent crime, and our heart and soul hurt terribly and somehow, we need to move through all of that hurt if we are to continue to live and face a big challenge we’ve never had to face before. It helps to think about what we may have learned along the way so far and if that can help us now. Life is no fairy tale. But each of us has an interesting story.Guy Tillson, Hospice Chaplain, Pemi-Baker Hospice & Home Health, Plymouth, NH

The next six-week session, grief and bereavement group “The Journey Through Loss and Grief” offered by Pemi-Baker Hospice and Home Health will begin on Tuesday, September 10, 2024, at 10:30am in the upstairs art room (accessible by elevator) at the Plymouth Regional Senior Center. Please arrive before 10:15am to get a parking slip for the dashboard of your car from the receptionist at the front desk. There is no cost for participation. We are experiencing increased demand for participation. Please contact Guy Tillson, Hospice Chaplain, via email at gtillson@pbhha.org or by phone at 603 536-2232 to pre-register.

Raising Awareness About Diabetes and Home Health Care Solutions

July 30, 2024

Pemi-Baker Hospice & Home Health: Raising Awareness About Diabetes and Home Health Care Solutions.

Pemi-Baker Hospice & Home Health is committed to raising awareness about diabetes and providing comprehensive care to those affected by this chronic condition. With the number of diabetes cases rising, understanding the disease and knowing the available support options is more important than ever. Pemi-Baker Hospice & Home Health Diabetes patient Testing blood sugar

Diabetes is a serious health condition that affects how your body turns food into energy. Most of the food you eat is broken down into sugar (glucose) and released into your bloodstream. When your blood sugar goes up, it signals your pancreas to release insulin. Insulin acts like a key to let the blood sugar into your body’s cells for use as energy. With diabetes, your body doesn’t make enough insulin or can’t use it as well as it should. When there isn’t enough insulin or cells stop responding to insulin, too much blood sugar stays in your bloodstream, which over time can cause serious health problems, such as heart disease, vision loss, and kidney disease.

For those who qualify, home health care can be a crucial resource in managing diabetes. Our skilled healthcare professionals at Pemi-Baker Hospice & Home Health provide personalized care plans to help patients manage their condition effectively at home. Our services include:

  • Education and Training: Our nurses educate patients and their families about diabetes management, including medication administration, blood sugar monitoring, and lifestyle changes.
  • Medication Management: Ensuring patients take their medications correctly and on schedule.
  • Nutritional Guidance: Our nurses offer guidance and advice to help manage diabetes through diet.
  • Regular Monitoring: Frequent check-ins to monitor blood sugar levels and overall health, allowing for prompt adjustments to care plans as needed.
  • Support and Counseling: Emotional and psychological support to help patients and their families cope with the challenges of living with diabetes.

For patients who qualify for hospice care, our team provides compassionate end-of-life care that focuses on comfort and quality of life. Our hospice services include pain and symptom management, emotional and spiritual support, and assistance with advanced care planning.

Don’t wait to ask about how our compassionate staff can help you or your loved ones living with diabetes. To qualify for home health care, a person must have a physician’s order and demonstrate a need for skilled care that can be safely provided at home. Announcing New Grief & Loss Support Group Series Starting June 25

June 17, 2024

Pemi-Baker Hospice & Home Health will begin its next Grief & Loss Support Group on Tuesday, June 25, 2024.

The topic for this six-week series is “Does It Ever Stop?” This is a question that comes to most mourners sometimes as they make their way through coming to terms with the loss they have endured. Free Grief & Loss Support groups offered at Pemi-Baker Hospice & Home Health, Plymouth, NH

The late Queen Elizabeth II once said, “Grief is the price we pay for love.” To the extent that we have invested and shared our love with another, we are likely to feel a comparable depth and soreness of sadness in losing them. A simple answer to the topic question is “No” because most of us believe that love is eternal, so the ache of grieving may endure for long stretches of time.

What can stop is the intensity of the hurting, so that the mourner may eventually come to an emotional space that is not as debilitating or devastating as when freshly felt. There are no shortcuts or easy steps toward arriving at this place. Grief experts do speak of such notions as acceptance, letting go, resolution, and finding meaning. We will look at and examine these ideas while continuing to assure that our group is a safe place for anyone regardless of where they find themselves as they move along in processing their grief.

Those interested in attending must pre-register by contacting Guy Tillson, Bereavement Coordinator and Hospice Chaplain. He can be reached by email at gtillson@pbhha.org or by phone at 603 536-2232. He will address any questions or concerns you may have. There is no cost to participate in the group. We meet on Tuesday mornings, beginning on June 25, 2024, at 10:30am, in the upstairs art room (accessible by elevator) at the Plymouth Senior Center. We hope you will join our safe and supportive series navigating the journey of grief.

The Rewards of Being a Hospice Volunteer

While I was familiar with Hospice for many years, I only have been a hospice volunteer with Pemi Baker Hospice and Home Health (PBHHH) since October 2021.  When I retired, I began searching for volunteer work that would allow me to give back to my community.  A newsletter from PBHHH arrived in my email announcing training for hospice volunteers.  My family had experienced the need for hospice twice, once in Ohio and again in Florida.  The relief my family and I felt when the hospice volunteers arrived was beyond measure.  I want to bring the same kind of assistance to others.  PBHHH given me that opportunity by providing professional training with knowledgeable nurses, doctors, social workers and other health care professionals.  Their guidance has enabled me to meet the challenges of being s hospice volunteer, and I am very glad that I did it.

A special thanks goes to Macayla and Colleen, who visited her on a regular basis, and to Lauren who was helpful in connecting us with state services.

“Everyone is so understanding supportive and kind.”

From the director to the nurses to the health care workers.  So cheerful, helpful and accommodating.  This is our second time receiving assistance from Pemi-Baker and we are very lucky to have found this amazing group!  Thank you for your excellent care and compassion.

“Pemi-Baker Hospice services provided kind, professional, competent services every step of the way-and even vaccinated us all for Covid/19!””

Our primary nurse, Fred, was outstanding, and others who filled in (Cathy, Penny, Colleen) were wonderful and a credit to your organization. Fred, you were amazing, not only for mom but for my husband and I as well. I will forever be grateful for your support, wisdom, humor and kindness, all along the end of life journey for mom. I am so deeply thankful that circumstances conspired such that mom was able to die at home, and your role made that time as seamless as it could be made.

Your courage during Covid-19 does not go unnoticed

Hello. We just wanted to take a moment and let you know how much we truly appreciate all that you do for us in these trying times. You're commitment to the health of our community is a special thing, and we know that you are working under stressful conditions due to the Covid 19 crisis. We realize that in order for you to treat those who may be ill, you are putting you're own lives and even you're own families lives in danger, and that is truly a courageous and honorable thing to do. We want you to know that it does not go unnoticed. Thank you all for all that you do!
