- Physician with Palliative and Hospice Care Experience -15-20 hours per week
The Physician participates in weekly interdisciplinary team meetings, assists the Hospice/Palliative Care APRN with collaborative discussions, completes CMS hospice required documentation, participates in call rotation. Works collaboratively with the Hospice/Palliative Care Team. Performs required MD visits. Hospice and Palliative Board Certified physician desired but not necessary. Interest in Palliative and Hospice Care a must.
- Per Diem APRN Hospice/Palliative Care
The Hospice/Palliative Care APRN ensures the quality and safe delivery of hospice care in compliance with Agency policy along with State and Federal Regulations. This position works closely with the Hospice/Palliative Care Team making palliative care, hospice, and advanced care planning visits along with assisting in phone triage related to hospice and palliative care patient’s symptom management. Participate in weeknight and weekend call rotation.
Must hold a current State of NH APRN license. Must be CAQH registered (Current or within 6 months of hire.) DEA license required. National certification as a hospice nurse practitioner (HPNA, CAPC) or other appropriate credentialing within a year – Preferred. Interest in Palliative and Hospice care a must.
- Home Care RN- part time or full time, 30-40 hours

You must be a currently licensed RN in the State of NH. A valid driver’s license as well as an insured vehicle and willingness to travel in all types of weather conditions are also required.
- Home Care Licensed Nursing Assistant (LNA)- 30 hours
