- APRN Hospice/Palliative Care- per diem
The Hospice/Palliative Care APRN ensures the quality and safe delivery of hospice care in compliance with Agency policy along with State and Federal Regulations. This position works closely with the Hospice/Palliative Care Team making palliative care, hospice, and advanced care planning visits along with assisting in phone triage related to hospice and palliative care patient’s symptom management. Participate in weeknight and weekend call rotation.
Must hold a current State of NH APRN license. Must be CAQH registered (Current or within 6 months of hire.) DEA license required. National certification as a hospice nurse practitioner (HPNA, CAPC) or other appropriate credentialing within a year – Preferred. Interest in Palliative and Hospice care a must.
- Home Care RN or LPN- Full time, 40 hours
Pemi-Baker is seeking LPNs and RNs to join our fabulous and friendly team. This position includes seeing both home care and hospice patients. Must be proficient in venipuncture, Foley insertion, ostomy and wound care, IV’s and ports, end of life and post mortem care. Home care and hospice experience is a must. Physical Therapist-Full time, Part time
As a Physical Therapist you will have the opportunity to work with patients one on one in our facility with openings in our outpatient program. You will be able to focus on ADLs and cognition. You will develop individual care plans for each patient allowing them the opportunity to function more independently at home. You will provide physical therapy evaluation and treatment to patients upon appropriate medical referral and/or as medically necessary. You will perform related duties of documentation, communication with physicians and other ancillary services and patient charges. This position allows for a flexible work schedule with a great atmosphere, supportive and friendly staff.
Must be detail oriented, well organized, communicate well with patients and be a team player. Licensed in the state of NH. New PT graduates are encouraged to apply, though previous experience is preferred.
- Hospice Chaplain-Part time/Full time
The Chaplain accompanies the patient and family through the final phase of life with interdisciplinary support to achieve a good death experience by redefining hope. Additionally the chaplain is accessible to the Homecare patients of the agency as a whole on a case by case basis. Coordinates and supervises the bereavement services provided by the Hospice program. Plans and facilitates bereavement support groups. Plans annual memorial service in collaboration with the Hospice Team. The Chaplain is accountable to the Hospice and Palliative Program Supervisor.

Social Work-Per diem/ Part time
PBCH is looking for either a MSW or BSW to perform home visits for our home care, hospice and palliative care patients. BSW required. MSW preferred 20-30 hours. Day shift. No weekends, nights or call
Must be a team player with exceptional people skills, be extremely customer service oriented, flexible, detail oriented, well organized, a solid knowledge of computers, excellent attention to detail, communicate well and be able to multi task. The social work position would entail patient home visits to assist with gaining access to community resources as well as counseling related to illness, behavioral health issues, and end of life issues. Palliative Care social work would include in-home or hospital consultations around advance directives, POLST, and goals of care for chronically ill and terminally ill patients. For hospice patients, the Social Worker also provides bereavement support/brief counseling services. Experience with home care, hospice and electronic medical records for SW documentation is preferred. Additional tasks to be included but not limited to volunteer tracking, organization, orientation, assisting in expanding the Agency’s Volunteer Program.
